WP Cost Estimation & Payment Form not working anymore

Hi there, on my website I’m using WP Cost Estimation & Payment Form for two forms. One of them is still working while the other one is not loading on frontend anymore.
I’ve created two pages to showcase the issue.

Did you assign any conditions to the form? Unfortunately, we are not familiar with the plugin, and as stated in our support policy, we do not provide support for third-party plugins. You may need to contact the plugin developers for further assistance.

there are no conditions in place.
Isn’t the plugin part of the theme bundle anymore?

Sorry for the confusion; the said plugin is still a part of the theme bundle. I have checked both the pages, and the forms are loading correctly. Can you please provide any screenshots marked with the issue or any video that helps us to recognize the problem?

For some unknown reason, it wasn’t loading as expected. If you encounter the same issue again in the future, please let us know. We’ll do our best to replicate and investigate it further.
