Javascript Forms not showing up on mobile

I used the raw content element to add javascript forms to these pages:

https://any.0ef.myftpupload.com/connect/
https://any.0ef.myftpupload.com/visit/

They do not show up in the chrome browser on my mobile phone.

However, the footer form for the email news & updates sign up does display on my mobile.

What can I do to change the display for the connect and visit pages?

Thanks for writing in! When I check those pages, there is a JS error on the page. It could also be caused by a plugin conflict, amongst other things. Please check out this troubleshooting article here and follow the instructions for the following sections (where appropriate):

Testing for Plugin Conflict
Theme Update
Child Theme
CSS/JS Customization
Disabling Cache
Disabling CDN

If nothing is helping, provide us access to your site so that we can check your settings. Please create a secure note with the following info:
