CS Forms Bug: Submission Conditions

A client told me, he’s not receiving form submission on his website. After some research I noticed, form submissions aren’t stored at all after their submission. The only 3 submission were mine from moths ago, while testing the form. After removing the “Submitted without Errors”-condition on the form and the success message, it finally worked again.

I then created an entirely new form on a new site to rule out any possible configuration errors on my side. I followed this tutorial step by step and used the “Contact with Accept” template that’s shipped with MAX. But even then, the condition wouldn’t work. The form is submitted, but the success message is not showing up at all.

Any ideas what could cause this?
