    Follow-up:

    My form is still not showing on larger monitors. The form looked great on my 13″ laptop, but is still invisible on my office monitor, which is around 22″.

    Additionally, the JS code change submitted by Rue earlier in this topic has made it so my form is no longer fixed to the header on mobile/tablet devices. The prior code did exactly what I wanted, but there was a supposed error caught by Rue.
