Thanks for writing in! Most probably your plugin is corrupted or missing some dependency files. Could you please try re-installing your plugin and see if that helps.
Once you download all files from your Themeforest account,
You should be able to locate tco-email-forms.zip plugin file under x_package/extensions/.
