It appears my vaildation screen options are not loading properly. Screenshot attached. Everything seems to be working okay so far, however, I feel like the template options I can pull samples from maybe not be all there. 
Can you check to make sure Cornerstone is at the latest version? This could happen if X is updated independently from Cornerstone.
I did the updates. I was not updating Cornerstone because it was telling me the license was not valid. Now it is good!
