Recent post feature images only showing on hover

We updated our cornerstone and now none of the feature images are displaying properly:

Instead it just displays a green box and the default icon.

I already tried clearing the style cache and disabling all the 3rd party plugins and nothing is working.

This issue was addressed on the latest update release for X 6.1.3, Pro 2.1.3 and Cornerstone 3.1.3.
