Problem embedding js form

I’m using the raw content container to insert the following form embed code but on the page it outputs both the form and a link to the javascript just below the form! help! here’s the page: https://equalityny.org/sign-up/

I tried copying the actual js into cornerstone and nothing worked.

Please try adding the code in Clasis Raw Content or Text element and see how it goes.
