Not sure why this happens, but usually when activating E&P form builder, it also creates 5 demo forms. This time it does not create Layered Image & Single step, Credit simulator and Hotel room reservation. I mean it creates them, but does not import steps.
I tried on 2 diffrent servers and 3 wp installs.
I created an account for you on clean install, please can you check it or simply attach the zip file with exported forms from working plugin.

Please be advised that the Layered Image & Single Step and Hotel Room Reservation forms do not have any steps. It is using the last step in the form settings.
You can deactivate the plugin, delete it completely and re install it again. The demos should be imported once you have installed and activated the plugin.

Oh ok, tried it but still nothing. How do I get it to look like https://prnt.sc/owg21w because my demo only shows https://prnt.sc/owg2ls , not sure how to add images and select fields that change. I did check the documentation but there was nothing.
Can you maybe post a screenshot or attach exported form file for layered images, that includes the fields in last step?

Simply add a step and edit it adding your own fields and design.
You can also find the Last step in the tabs if you want to edit the fields in it.

No my problem is I don’t know how to create “Layered Image & Single step” from scratch. Do I create a step, then put two select fields in them? How do they switch the images based on what is in the select? Do I create a rule? Should I even create a step, maybe I need to create the fields in last step since I want “Layered Image & Single step”.
My problem is my demo is not creating me a proper “Layered Image & Single step” in forms list to learn from. I only have email field in my demo, nothing else is created. I need a new export of “Layered Image & Single step” or simply a screenshot of “Layered Image & Single step” last step fields and the content of those fields.

Is there a reason why you don’t want to attach the exported demo forms?
Can you login to my site and reinstall the demo (because my plugin is not working), so that I would have exactly this demo http://www.wordpress-estimation-payment-forms.com/layered-image/ in my forms list with the 2 select fields and demo images of walls.

Sorry for the confusion. The Layered Image & Single step demo is not importing on my end in all test servers as well so it could be that the said demo is only available for Standalone WP Cost & Estimation license holders or it could be a bug. I’ll post this in our issue tracker for clarification.
Setting up a form is outside the scope of our support so for now, you’d need to learn how to use the plugin. The trick to replicating the demo is to setup a Select field and Layered Image items in your step. Creating steps, items and conditions are clearly written in the documentation.
You then just use the plugin’s condition feature to show the images conditionally.
