Pro Theme - Controling Header & Footer for a particular page

I have an opt-in form that I need to show to prospects, but having regular Header for this particular Landing Page is not feasible. So I used Page type BLANK - No Container - No Header - Footer

I got this result:

I would like to be able to define a header specifically to be shown in this type of Opting Forms, Otherwise I will have to include the Top branding information as part of the page itself and copy to every Optin Form that I have…

I´ve managed to create a Pro Header with only my logo centered. Now I am trying to find a way to use this Custom Pro Header inside the page

I´ve got it. I needed to assign a page to the Pro Header…

I was thinking the other way around… and not finding a way to assing the header to the page…
