Page specific feedback form

Hello there,

My client would like a form with which they can provide feedback on the specific page they happen to be on at that time. Is this possible with either Contact Form 7 or WP Forms?

The client is hoping to replicate the functionality here: https://g-care.glos.nhs.uk/

Thanks for reaching out.
You can use the Modal element and add the Contact Form 7 or WP Form into it to get the feedback for the page you want.

Is it possible to change a label using a condition? For example, when the user selects Feedback on page content from the dropdown the content label changes to Page you are commenting on:

Unfortunately, that is not possible to change the form field labels.
