How to create form as the E&P forms builder sample

I can create step by step form. But I can’t create as below contact form. The form always show next step and successed messages and summary information that should not be displayed. How to do?

Then as for the form layout, you can change that for every form field that you add.

Hello Steve,

The maximum items per row depends on the items you have assigned as columns. If you only have two elements assigned as columns then you’ll have two columns.
