Help with convert plus submit form not displaying mail and name fields

Hi… the convert plus forms just stoped working… they are displaying all rigth but without the imput fields… I’ve checke it and everything is in place: the html form is in it’s place and is workig; I have everything updated, and I can’t seem to know what is happening… can you help me?

Thanks for reaching out.
I have checked your website but didn’t find any Convert Plus coming up on the Home Page. I have also checked the Popups from WordPress admin dashboard but didn’t recognize the exact form where you are having this problem. I would suggest you assign the popup to any page and share the popup name and the page URL for further investigation.
