Gravity Form Issue in Footer

I’m having with gravity forms in the footer of my site. When I click submit, it doesn’t seem to work. I just redirected back to the top of the page. I can’t even really fill in the fields. If someone could take a look, I would really appreciate it!

Please note that Gravity Form is a 3rd party plugin and it’s difficult for us to support the same. If the issue is not resolved, please get in touch with plugin developer support team for more help.
