Form cutoff on live site

I used an external CRM site (pipedrive) to create a form. The site gives me the following code:

div class=“pipedriveWebForms” data-pd-webforms=“https://pipedrivewebforms.com/form/06f7ae72fc02547a4897e645e97830122385001”></div

When I use that, the only loads the first few lines of the form and the rest you can only see if you tab through the form. The site is monkeybiznessfranchising.com/contact_us.

How do I get the site to display the complete form?

(note, this is on Chrome. On Edge, it doesnt show up, just the link to the .js)
