Is it possible to have a navigation icon lke in the screen shot that stays at the top of page as the screen is scrolled on mobile devices. This site was done with xtheme so was it custom code or can xtheme do that as is?

You can do this with custom CSS code if you are using X theme.
If you are using Pro theme that can be done without custom code.
In header builder of you can create a sticky header with transparent BG and the menu nav.
