Email Form Not Working

I have connected my mailchimp and created a form but I can’t get the form to show up in the email forms list in cornerstone to show on my homepage.

I added the login info in a secure note. The issue is still there. Which settings should I change in my mail chimp account?

The email form element is no longer available within the builder, your content is probably from the old version where it’s still available. Please use email form elements within the text or raw element instead.
