Easy Bottom Right Contact Slide?

Does anyone have a suggestion that would make it easy to implement something like this: http://talkerscode.com/webtricks/demo/demo_easy-feedback-form-on-page-bottom-using-jquery-html-and-css.php in Pro? I would have thought that Content Dock or ConvertPlus would have some options that would create this type of effect… but no. I also would have thought there would be some other WP plugin out there that would just handle this but I can’t find one.

I could install and customize the code found in the example I am providing. Won’t be a problem for me. But I’m just trying to see if anyone has run across a simple plugin that would just do it well. I just want to place a Gravity form shortcode in there and give the user a way to contact us in the bottom right (read: not a floating tab on the right which is what all of the plugins I found do) that they can open and close at will… just like the example.

Any thoughts? Ideas, Suggestions?

Well of course I’ve already looked in the wordpress repository. I’ve looked all over. Does anyone here have any direct experience with one that does specifically what I’m asking for?
