E&P forms not displaying correctly

Hey there, I have searched and searched and I cannot figure out why my E&P cost estimate form will not load. I just get a bunch of text and none of the scripting.

I will include a link to my site in the secure note, and you will see I have tried to place the form in a classic raw content, new V2 content area, and as a modal popup using a global block, and none of these elements are working to display it.

Thanks for writing in! Could you please try disabling your 3rd party plugin and re-test this issue again. If you have added any custom Javascript, please remove them temporarily as well.
