Cornerstone Broken(?) - Some options not showing up

Fresh install on a staging server is having issues with some corner stone options. Here is a screenshot to illustrate the issue.

I just confirmed that X theme and cornerstone are both up to date:

X theme - Version: 6.4.6
Corrnerstone - Version 3.4.6

To fix the problem, under X > Settings > Permissions > User Preference > Advanced Mode select Always On.
