Convert Plus modal and slide in buttons will not trigger submission/notification

Hello, I have an active modal and a slide in with convert plus and after filling in the form and clicking the submission button, nothing happens. I have made the modal live and gone through all of the design/behavior/submission options to see what might be the problem. I have notifications on, and set the modal/slide in to display to everyone who sees the site. The URL for the modal is: http://interstate-48.com/services/dispatch-service/

Always make sure to update both X and Cornerstone to the latest versions to avoid such conflicts and potential errors.
