Convert Plus form not collecting all field data

Good day Support,

I’ve set up a Convert Plus Modal Popup with a form that collects data before allowing users to download a PDF document.

The form has fields for First Name, Last Name and Email. The Convert Plus connects is collecting the email address data but not the First Name and Last Name field data.

How do I need to configure the form correctly to collect this data as well?

It sounds like an issue with your form setup.
