Contact form not pulling information through

I have a contact form http://macsbathrooms.com/request-a-home-survey/

However, when ever it is being filled in it is not talking all the details across?

You will have to indicate the form fields that the form will send to the recipient in Contact > Contact Forms > Edit Contact Form > Mail settings.

Kindly set the fields in the Message Body option.
