Contact Form Design Block Template

I am utilizing a design block template and cannot get the contact form to work with Contact 7 plugin. I created the Contact form in the plug in but when I select it in Cornerstone nothing happens.

Could someone look into this for me?

Are you referring to the layout/styling of the form not the same on the Youtube tutorial? Please see the video description there is a link there where you can copy the Form structure and CSS styling.
