Advanced Custom Fields: User field data into a form

Hi,
This may be more advance then you can help with but I have a client looking to have a check list of items in a users area, that when one or more of these items gets selected by the administrator, the checked item gets added to a form for email.

I’ve used Advanced Custom Fields (ACF) to add some Checkbox fields to the users area but I’m having no luck moving that data to a form. I watched this video tutorial https://www.youtube.com/watch?v=9NDlnEIBTUg but I’m still not getting things to work. Even with global blocks.

I’m sure I’m not explaining this correctly as I’m just wrapping my head around it myself.
