Act-On forms work only on some pages

Hello. We’re using the embed form on all of our pages. We’re using Act-On as our form generator as that is where our CRM is.

On one page, the form works. Take the code as-is, copy paste it into another page using embed form and the form no longer works. It’s completely sporadic. Some pages it works, other pages it doesn’t. I need to launch this site ASAP. Please help.

Contact page = Working
Compliance page = Not working
Same code and process on both pages.

Thanks for writing in! When I checked both pages, the form is not displaying on my end. And I am seeing this JS error in the console:

This could mean that a 3rd party plugin is causing the issue. Please do the following:

• Testing For Theme Related Issue
• Testing For Plugin Conflict
• Theme Update
• Child Theme
• Css/Js Customization
• Increasing Php Memory Limit
• Disabling Cache
• Disabling Cdn
• Permalinks
• Version Compatibility

And I also noticed that you are using Cloudflare. Please log in to your Cloudflare account and purge your site cache before testing your site again. It would also help if you turn ON the “Development Mode” in Cloudflare while you are still troubleshooting the issue.
