RupokMemberHi there,
It seems you form is not configured properly, perhaps you changed the format. Would you mind providing us with login credentials so we can take a closer look? To do this, you can make a post with the following info:
– Link to your site
– WordPress Admin username / password
– FTP credentialsDon’t forget to select Set as private reply. This ensures your information is only visible to our staff.

RadModeratorHi there,
You have to use the input name on your email content. Example,
If your text field is like this
[text* your-name id:ad placeholder "Naam"]Then in your email, it should be like this
From: [your-name] <[your-email]>It’s because your text field name is your-name hence [your-name].
Now, your text field is currently like this
[text* ad id:ad placeholder "Naam"]Which means, the text field name is ad, hence, in your email setting it should be like this
From: [ad] <[your-email]>It’s just matching the field name and bracket name in your email message. The same rule applies for all form fields.
Hope this helps.
