Thanks for the updates! Both Mailchimp and Google Maps is using Javascript (JS). Once there is a JS error generated by any of the active plugins, it could affect a plugin such as Mailchimp or any other plugin too. This is in relation to your question. By the way I have tested your site specifically the mailchimp form on the page. I could not replicate the issue. Your mailchimp form is working. This is my test result: http://prntscr.com/cs03ob

I have wrapped your Mailchimp CSS modifications in Appereance > Customizer > Custom > CSS in a media query:
@media (min-width: 769px) { }And now it looks great on portrait tablets and mobiles
