    Hi There,

    Thanks for writing in! That form is create with a plugin named Contact Form 7, its free and it works with X|Theme very well.

    but how do I get it on a page with nothing but a form?

    Hello There,

    Thanks for updating in! Normally, when you create a page, there is a header and a footer. If you want to have a page with nothing but the content and the form, simply edit the page and use the Blank – No Container | No Header, No Footer page templates.
