Would you mind providing us with login credentials so we can take a closer look? To do this, you can make a post with the following info:
– Link to your site
– WordPress Admin username / password
– FTP credentialsDon’t forget to select Set as private reply. This ensures your information is only visible to our staff.
In the meantime can you try adding the contact shortcode on a Text element instead of the RAW content element.
And please confirm that your contact form plugin is active.

Thanks for updating. I tried to place this in a default editor and it doesn’t work either. Then I noticed you don’t even have Contact Form activated from Jetpack – http://prntscr.com/becwhq
I have activate this and the form shows fine. However I am not sure about the other code (WuFoo embed code) as I am not sure if that requires any plugin or not since it’s a shortcode and you should have a plugin for this that I can’t see.
