The photo & text surrounded by the shortcode disappears sometimes – it seems random. It’s also spacing the text in a strange way… without any spacing code…
Can you help?

I’ve updated all plugins and my theme… that seems to have sorted the forms and pictures from disappearing, but there is still a large gap and no ‘gap code or code in the text editor to create it. Can you help me to understand where these gaps are coming from?

You could try testing for a plugin conflict. You can do this by deactivating all third party plugins, and seeing if the problem remains. If it’s fixed, you’ll know a plugin caused the problem, and you can narrow down which one by reactivating them one at a time.
If that doesn’t help please provide wordpress admin login in private reply so we can take a closer look.

It is most likely there is some syntax mistake on your code. I could not find anything while looking at it, but once I deleted the one that was not working and repeated the one that is working, all the modals inclusive the bottom one worked fine.
So please remove the one that is not working and replace with a new one using the working code as a reference:
