Thank you providing the information. It turns out that in the settings of Yoast SEO plugin, there is an excess % character. I already removed it to save time. You can just check it here for reference (http://prntscr.com/crz7ok). As point of reference, you can check this in SEO > Titles & Metas > Post Types.
I have removed it already and cleared the caching plugin. This issue should be resolve now.
