I fixed the javascript error and it’s originating from your custom code.
How should the Woocommerce Upload Files plugin works?
I also checked the Woocommerce checkout manager and it’s working, the upload functionality is on the top of the checkout page.

Maybe it’s not compatible anymore, their latest version was half years old. And your Woocommerce version is 2.6.14. You may try another plugin or contact them about version compatibility.
