I can’t change the permalinks now. where should i setting?

I create a wufoo form. and i use the JavaScript Version to add it into my page.
but there are two problem.
1)the form lost the theme made at wufoo.com . just showed the white background.
2) i use the “Blank- no container/header.no footer” template, but the form covering my pages, and the table displayed on the left , not in the middle.

what wrong?

#1 You need to press on Edit button at the top of page below page title.If you have already done that and it’s not working then provide us with login credential .

#2 Please change page template to “Blank-Container/Header.No Footer”

Please change your theme to any other theme and check again if the permalink Edit button appears or not.

As i deactivated all 3rd plugins the Edit button appear (see:http://prntscr.com/4q5u1r)
Please try to deactivate plugins(except those are recommended by X) one by one to find out which plugin cause the issue.
