I’ve been watching this tutorial of Email form extension, but it doesn’t give me what I need.
I would like to create a custom from with customized action url to it (like attached one).
Is that available by X Theme? Or should I do that manually as a short code? Because as you know wordpress will delete all form inputs if I did that from wordpress editors.

Thanks for writing in! I am not sure what you exactly need so can’t suggest anything. If the available feature of Email Form extension doesn’t help then you can check the ConvertPlug extension or check for other third party plugins.

Thanks for your response! I didn’t know that I can show the modal as an inline short code which is generated via ConvertPlug extension, that did the trick.

After digging more deeply with inline forms using ConvertPlug, I didn’t know from where I can assign a custom action URL for the generated forms, I think I can’t do that? And if I do a custom forms, I’ll lose the generated design: http://screencast.com/t/cSIczJMzy9G
