ConvertPlug and Mailchimp

I’ve been trying to get the ConvertPlus to work with no luck. I’ve read other posts with a similar issue and have tried suggested fixes with no help. I’m getting a message “There appears to be an error with the configuration” when submitting the form. I’ve disabled all other plugins, I’ve tried with and without double opt-in.
And when not logged in as an admin, the form will come up briefly, then disappear.

I’d really like to use ConvertPlus on all my sites (I have a ton of licenses with you all and use you for every client). I’d really appreciate any help you can offer.

I’m currently only targetting one page while testing:

Everything is up to date as well. All themes, plugins, wordpress, and PHP.
