Convert Plus fields not showing up

Hello,

I am trying to create a modal in convert plus and regardless of type the form fields never show up. I have attempted multiple new modals, & deactivating all other plugins to no avail. Any input as to the potential cause to the problem? Thanks!

Can you confirm that you are fully updated? (Theme and Plugins)

You can find the latest version numbers are

Pro 2.0.4 - If you are using pro

X 6.0.4
Cornerstone 3.0.4
Converplus 3.2.2

Then you can compare them to what’s installed on your site.

In the meantime, can you please try enabling your WP_DEBUG mode on your wp-config.php file to see if you’re getting any PHP errors. Also make sure that you’re using PHP 5.6.x or later to avoid incompatibility issues.
